Making Appointments
If it is convenient, please use the web form on the next page. Please include in your email 2 or 3 requests with the date and time of day, and we will do our best to accommodate you. Let us know the best way to get back to you, either by phone or email.

Please include your Insurance Type and Insurance ID Number in your appointment requests. The length and complexity of most office visits can be anticipated beforehand. Please help us to run on time by giving us details about your visit when you schedule your appointment. For instance, let us know if you need a routine annual or semi-annual check-up, with no interval problems to report, or if you feel you need a longer consultation and tests for an ongoing or new medical problem. Or let us know if you are coming in for a brief follow-up visit after a procedure. Visits that require longer time include pre-surgical consults, vulvar pain consults, and second opinions.
Please bring in all previous records, sonograms, and test results. For new patients and those transferring from our previous practices, please click on and print the record request form, and send it to your previous health care providers as soon as possible. Request that a copy of any relevant records be sent to you, and then bring them with you to your visit. This way we can abstract them for your electronic medical records and then return them to you for your ongoing safe-keeping.
Finally, it would be helpful to receive 24 hours notice if you need to change or cancel a scheduled appointment. Our goal is to minimize wait both for an appointment and during your visit with us.

If you need to send for records from other doctors for us to review, print this out, fill in your part, and fax or mail it to other doctors to get your records to us.
