number and email address if it is available). Although we
try to handle all requests on the same day, please allow two
business days so that you do not run out of your medication.

Insurance Type and Insurance ID Number in your appointment
requests.
The length and
complexity of most office visits can be anticipated beforehand.
Please help us to run on time by giving us details about your
visit when you schedule your appointment. For instance,
let us know if you need a routine annual or semi-annual
check-up, with no interval problems to report, or if you feel
you need a longer consultation and tests for an ongoing or new
medical problem. Or let us know if you are coming in for a
brief follow-up visit. Other visits that require longer
time scheduled include pre-surgical consults or second opinions.
Please bring in all previous records, sonograms, and test
results. For new patients and those transferring from our
previous practice, please request that a copy of any relevant
records be sent to you, and then bring them with you to your
visit.
This way we can abstract them for your electronic
medical records and then return them to you for your ongoing
safe-keeping.
Finally, it would be helpful to receive 24
hours notice if you need to change or cancel a scheduled
appointment. Our goal is to minimize wait both for and
during your appointment.
